VALVe actually released an entire SDK for it's engine, with source code snippets, APIs (Application programming interface), map editor and full access to the base content of their games and also the source files for some of the maps used in the game.
This makes HL definitely number one as the most moddable. (not most modded, even though I think HL has held that merit several times as well due to how easy it is and the flexibility you have).
And if you were accepted as a licensee of the game engine you get even more goodies I have heard. So there is a thin line between making mods and actually using the game engine itself to make new games.

UT99 fans, keep tryin - but I'm still at a loss where anybody changed the game into anything other than UT99 with different weapons, gravity, etc. :) I hardly consider a new map or set of weapons to be earth-shattering...what did it bring that was original or totally different from its boxed game?
Not trying to be argumentative, but that's what *I* looked for when making this list. I'm not saying it doesn't deserve a spot, because I think it's up there - but I don't see how UT99 modded is anything...well, more than UT99. So show me!!
What makes UT99 special is how easy it is for anyone to create a map. UnrealED makes this very easy and this is why there are still a lot of people playing this game. Get on a server and open the map vote and see how many of them there are.
I understand your point, but I guess 'moddable' just doesnt mean the same thing for both of us. That's cool though, I know UT99 is the best game ever made on PC, and it will always be :D
Then again Hammer is probably even easier (when its not crashing left right and center)
I assume Quake4 wasn't included, because, tbh, its not modable at all, its built on the Doom3 engine and as such, is just as hard to do anything with
HL and HL2 have hundreds of mods, full conversions, and addition maps (you can look on wikipedia for a list)
Shame on you for not including Unreal Tournament. Every game with every release came with the editor which was used to create the game. So many thousands of maps have been created. Many more weapons, characters game types and etc. Actually with UT modding community directly affects the next game in a very big way. The biggest mods ussually become included in the next version.

Also I will never forget Operation Flashpoint and all the fun I had with it's community. Oh man good times. People made all kinds of games out of that one. From WW1 dogfight, WWII shooters, to racing and even life and train simulators. The actual OPF game was like an additional feature compared to the modded things.
isn't red orchestra built on the 2K3/4 engine ? (even though its now distributed over steam)
My favorite mod for UT is still Chaos UT, damn that can be fun :)
However, UT was still never really modded to the extent of HL, Quake or morrowind
